About the Role

Care Partners is seeking a highly skilled Behavioral Health Clinician to join our team. As a key member of our interdisciplinary team, you will provide comprehensive mental health services to older adults with complex psychosocial needs.

Key Responsibilities
  • Crisis Intervention and Management: Provide timely and effective crisis intervention and management to prevent hospitalization and ensure the well-being of our clients.
  • Assessment and Treatment: Conduct thorough assessments and provide individual, family, and group therapy to address the unique needs of our clients.
  • Case Management: Develop and implement comprehensive service plans to ensure the optimal care and support of our clients.
  • Interdisciplinary Collaboration: Work closely with our interdisciplinary team to coordinate care and ensure seamless transitions.
  • Program Development and Implementation: Assist in the development and implementation of programs and policies to support the Mental and Behavioral Health Team.
Requirements
  • Master's Degree: A Master's degree in Social Work, Counseling, or Psychology is required.
  • Experience: A minimum of 1 year of experience working with geriatric populations or in long-term care settings is preferred.
  • Clinical Skills: Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skills are essential.
  • Communication and Interpersonal Skills: Excellent verbal and written communication skills, as well as good interpersonal skills, are required to work effectively with clients and colleagues.
