About the Role

Care Partners is seeking a highly skilled Behavioral Health Clinician to join our team. As a key member of our interdisciplinary team, you will provide comprehensive mental health services to older adults with complex psychosocial needs.

Key Responsibilities
  • Provide crisis intervention and management to prevent hospitalization.
  • Conduct intakes and assessments to identify participants' mental health and behavioral needs.
  • Develop and implement individualized treatment plans, including therapy and case management services.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support.
  • Lead therapy groups and provide education to participants and their families.
  • Contribute to team meetings and case conferences to share knowledge and best practices.
  • Provide information and resources to the community as needed.
  • Supervise and mentor licensed staff, including LCSWs and MFTs.
  • Assist in the development and implementation of programs and policies to support the Mental and Behavioral Health Team.
Requirements
  • Master's degree in Social Work, Counseling, or Psychology.
  • 1 year of experience working with geriatric populations or in long-term care settings.
  • Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skills.
  • Excellent verbal and written communication skills.
  • Good interpersonal skills, including ability to work cooperatively on an interdisciplinary team.
  • Ability to work with diverse ethnic backgrounds and cultural orientations.
  • Bilingual ability may be required for certain positions/assignments.
