Full Time Patient Engagement Coordinator At BayMark Health Services, we prioritize a unique approach to patient care. This role is essential in supporting patients within our Residential Treatment Facility throughout their recovery journey. The Patient Engagement Coordinator (PEC) plays a crucial role in daily patient interactions by offering activity services, crisis support, and connecting with patients' diverse interests to enhance engagement. All interactions will be meticulously documented in the patients' Electronic Medical Records (EMR). PECs guide patients in maintaining appropriate behaviors, actively participating in their treatment, and reflecting on the benefits of their involvement in programming. They encourage attendance at all group sessions, medical appointments, and clinical meetings. Key Responsibilities:
  • Complete EMR documentation during the designated shift, ensuring clarity and timeliness.
  • Monitor patient behavior during group sessions, ensuring respectful participation and addressing any unexcused absences.
  • Review the patient handbook and assist in orienting patients to the program.
  • Facilitate educational and support groups for patients.
  • Assist in transporting PHP patients to off-site activities.
  • Provide individualized support to detox patients through daily meetings, reviewing programming requirements, and offering concierge services to enhance engagement.
  • Aid in rescues for patients discharging Against Medical Advice (AMA).
  • Be available for crisis intervention with patients.
  • Practice personalized coping strategies with patients.
  • Collaborate with the clinical department to plan and execute engagement activities throughout the month.
  • Document weekly the number of patients engaged, noting those who left AMA and those successfully retained in treatment.
  • Offer services based on specific skill sets to engage patients, such as physical fitness, arts, music, or gardening.
  • Coordinate activity services tailored to individual skill sets.
The Patient Engagement Coordinator assigned to concierge care will play a vital role in crisis intervention and community engagement, promoting positive interactions, reducing AMA risks, and providing additional support to patients struggling with treatment engagement. Responsibilities include assisting patients in attending groups, meeting appointments, participating in campus activities, and removing barriers to treatment. Minimum Qualifications:
  • High school diploma or GED.
  • Ability to thrive in a multi-cultural, community-focused organization.
  • Commitment to confidentiality.
  • Understanding of the agency's mission and culture, maintaining appropriate boundaries with staff and clients.
  • Previous experience with substance abuse and co-occurring disorders is preferred.
  • Employment contingent upon clear UDS and criminal history record.
  • Strong interpersonal skills.
  • Ability to lift supplies up to 25 lbs. to waist height.
  • Must be able to navigate up to three flights of stairs during a typical workday.
  • Relevant skill set and/or certification based on facility needs.
  • Successful drug screening and background check.
Salary Range: Generally starting at $20.00 per hour.
