Patient Care Coordination Specialist

Compensation: $65,000 annually
Overview:
VitalCheck Wellness is in search of a dedicated Patient Services Coordinator with expertise in phlebotomy to join our outpatient clinic associated with a global wealth management firm. This role presents a unique opportunity to contribute to the establishment of an innovative wellness clinic model.
Key Responsibilities:
- Engage with patients to review health history, diagnoses, and individual needs.
- Coordinate follow-up care and manage appointment scheduling and referrals.
- Serve as the initial contact point for patients.
- Provide education to patients regarding necessary follow-up care steps.
- Manage referrals, including communication with insurance providers to verify coverage.
- Accurately input and upload patient data into the system.
- Deliver exceptional customer service to all patients.
- Maintain a professional demeanor at all times.
- Communicate effectively with all clinic staff.
- Organize patient engagement activities and assist in event planning.
- Proactively create spreadsheets, resolve issues, and plan events.
- Collect vital signs and perform specimen collection accurately (phlebotomy, urine, etc.).
- Required: CNA/CMA/LPN or Phlebotomy Certification for California.
- BLS/ACLS or CPR Certification.
- Experience in a clinical environment or hospitality sector.
- Familiarity with medical terminology.
- Prior experience as a medical receptionist or assistant with scheduling responsibilities.
- Proficient in phlebotomy.
- Strong customer service skills.
- Ability to adapt to evolving situations.
- Highly skilled in spreadsheet applications (Excel and Google Docs) and Office Suite.
- Capable of troubleshooting issues independently.
- Proficient with technology and able to work autonomously.
- Must be fully vaccinated against COVID-19.
