About the Department:

The Empowered Community Opioid Project (ECOP) employs strategies from care coordination, case management, and population health to enhance health outcomes and resource utilization for specific populations and individuals. This focused approach is a hub-and-spoke community care model that organizes clinical service delivery assets into a network comprising a central establishment (hub) offering comprehensive clinical support services, and secondary communities (spokes) directing patients requiring more intensive services to the hub for treatment. Furthermore, it engages in active cross-sector collaborations with criminal justice, behavioral health, and medical teams to optimize resources.

About the Position:

The Community Support Specialist plays a crucial role within the ECOP Team. As a peer navigator, this position is essential in establishing a strong connection between participants and community partners (CSB, jail, others). The Community Support Specialist is part of the Community Site team (spoke site). This role continues to assist in developing recovery community models for cross-sector collaboration, engagement, and mentorship of the ECOP model, facilitating quick access to Medication Assisted Treatment (MAT) for individuals rapidly released from the carceral and legal systems.

The Community Support Specialist (Peer Navigator) fosters a one-on-one relationship with program participants who are at the beginning of their recovery journey. The Peer Navigator provides encouragement, motivation, and support. Responsibilities include peer navigation and coordination of Medication Assisted Treatment (MAT) bridges for the ECOP community site.

Responsibilities:

Peer Navigation for the Community Site (spoke):

  • Jail In-reach: Engage with incarcerated individuals facing opioid challenges to explain ECOP and enhance the likelihood of connection upon release. Provide resource suggestions for ineligible individuals;
  • ECOP Intake: Obtain consent from eligible participants and complete the intake process;
  • Peer recovery resource connection: Offer information on available resources for health insurance, COVID, medical, behavioral health, housing, transportation, etc.;
  • Community Building: Cultivate relationships with local agencies, advocating for participants. Assist individuals and communities in adopting healthy behaviors and social networks;
  • Collaboration: Work with partner peers for ongoing peer recovery support and coordination.

Bridge Care Coordination for Community Sites (spoke):

  • Medication for Opioid Use Disorder: Collaborate with the ECOP Bridge Team to facilitate rapid connection to MOUD and ensure local community provider intake;
  • Care Coordination: Partner with the ECOP Bridge Team to coordinate inpatient treatment and reengage with participants upon discharge;
  • Team Participation: Engage as a member of the integrated care team without being responsible for the care plan.

Data Tracking:

  • Participant Documentation: Record participant interactions and resource connectivity;
  • Monitoring Survey: Administer benchmark surveys;
  • Community Data: Gather data to identify social and community health needs and gaps.

Required Qualifications:

  • High school diploma or equivalent;
  • Certified Peer Recovery Specialist (CPRS) is required;
  • Lived experience of and in stable recovery from a substance use disorder, with significant sustained recovery time;
  • Ability to serve as a role model for recovery, wellness, and self-advocacy;
  • Disclosure of recovery experiences;
  • Completion of Virginia Department of Behavioral Health and Developmental Services (DBHDS) 72-hour peer recovery specialist curriculum;
  • Completion of Certified Peer Recovery Specialist (CPRS) within 6 months of hire;
  • Reliable transportation to meet participants and partners within the local Health District;
  • Ability to meet volunteer criteria at the local or regional jail.
  • Peer Support skills: Ability to utilize lived experience with addiction and justice involvement to assist others in accessing care and reintegrating after incarceration; maintain professional boundaries; use recovery journey to help others connect to services; maintain confidentiality; demonstrate strong written and verbal communication skills; build rapport and listen actively; role model recovery.
  • Technical skills: Proficient in typing and data entry; use of Microsoft Office, Excel, Outlook, Zoom; timely entry of notes into an electronic health record; and effective time management to complete tasks.

Preferred Qualifications:

  • Lived experience with the carceral and legal systems;
  • Bi-lingual (Spanish) speaking;
  • Experience in wraparound intervention as a peer specialist;
  • Collaboration on an integrated team with behavioral health, medical, or justice system partners.
