Hospitalist Care Facilitator: Hospitalist

The Hospitalist Care Facilitator plays a crucial role in maximizing Hospitalist efficiency by assessing, planning, implementing, and evaluating nursing care as directed by the Hospitalist Program.
Key Responsibilities- Coordinates patient discharges as ordered by physicians.
- Consults and collaborates with hospitalists, physicians, RN case managers, social workers, educators, and other healthcare personnel to coordinate effective and timely patient discharge.
- Ensures all discharge processes are completed as appropriate.
- Assists with admissions as directed.
- Serves as a liaison for physicians, communicating with all appropriate healthcare personnel, family, and patients.
- Maintains the sample closet and ensures appropriate inventories/supplies.
- Valid multi-state or State of Oklahoma Registered Nurse License.
- 5-7 years related experience.
- Working knowledge of general disease state management processes, bio-psychosocial model, and clinical pathways.
- Effective interpersonal, written, and oral communication skills.
- Ability to organize and prioritize work in an effective and efficient manner.
- Ability to utilize PC & software skills.
