Position Overview:
The Clinical Care Coordinator plays a crucial role in assessing, planning, implementing, and evaluating nursing care as guided by the Hospitalist Program, aimed at enhancing the efficiency of hospitalists.
Educational Requirements:
Completion of an accredited nursing program recognized by a state board of nursing, resulting in a diploma or degree.
Licensure and Certification:
Must possess a valid multi-state or State of Oklahoma Registered Nurse License.
Experience: - A minimum of 7 years in a related field.
Skills and Competencies:
Proficient understanding of general disease management processes, bio-psychosocial models, and clinical pathways. Strong interpersonal, written, and verbal communication skills. Ability to effectively organize and prioritize tasks. Proficient in using computer systems and software applications.
Key Responsibilities:
Manages patient discharge processes as directed by the physician.
Engages in consultation and collaboration with hospitalists, physicians, RN case managers, social workers, educators, and other healthcare professionals, as well as family members, to ensure timely and effective patient discharge.
Ensures all discharge protocols are followed appropriately. Assists with patient admissions as instructed. Acts as a communication bridge for the physician, interacting with all relevant healthcare personnel, families, and patients. Participates in meetings with hospitalists to discuss and coordinate patient care. Maintains inventory and organization of medical supplies.Decision-Making Authority:
Exercises independent judgment in planning and executing operations, making minor decisions within a complex technical or professional context.
Collaborative Relationships:
Coordinates the activities of others (does not involve direct supervision). Guides peers in performing similar tasks (does not involve direct supervision). Engages directly with patients and/or clients. Communicates with internal stakeholders via phone or in-person. Interacts with external clients through phone or face-to-face meetings. Collaborates frequently with healthcare professionals and staff, including individuals at the Director level or higher.
