Position Overview:
The Clinical Care Coordinator for Hospitalists plays a crucial role in assessing, planning, implementing, and evaluating nursing care as guided by the Hospitalist Program to enhance the efficiency of hospitalist services.

Educational Requirements:
Completion of the foundational professional curriculum from an accredited nursing school, verified by a state nursing board, and possession of a diploma or degree.

Licensure and Certification:
Must hold a valid multi-state or Oklahoma Registered Nurse License.

Experience Requirements:
  • A minimum of 7 years of relevant experience in a healthcare setting.

Knowledge, Skills, and Abilities:
Proficient understanding of general disease management processes, the bio-psychosocial model, and clinical pathways. Strong interpersonal, written, and verbal communication skills. Ability to effectively organize and prioritize tasks while utilizing computer and software skills.

Key Responsibilities:
- Coordinate patient discharges as per physician orders.
- Collaborate with hospitalists, physicians, RN case managers, social workers, educators, and other healthcare professionals to ensure timely and effective patient discharge.
- Ensure completion of all discharge processes as required.
- Assist with patient admissions as directed.
- Act as a liaison between the physician and relevant healthcare personnel, patients, and families.
- Meet regularly with hospitalists to discuss and coordinate patient care.
- Maintain inventory and organization of medical supplies.

Decision-Making Authority:
Utilize independent judgment in planning operations and making minor decisions within a complex healthcare environment.

Collaborative Relationships:
Coordinate activities among healthcare team members without direct supervision. Engage directly with patients and customers, and collaborate with internal and external stakeholders, including healthcare professionals at various organizational levels.
