Position Overview:
The Hospitalist Care Facilitator plays a crucial role in assessing, planning, implementing, and evaluating nursing care as directed by the Hospitalist Program, aimed at enhancing the efficiency of hospitalists.

Educational Requirements:
Completion of the fundamental professional curriculum from an accredited nursing school, with eligibility for a diploma or degree.

Licensure and Certification:
Possession of a valid multi-state or Oklahoma Registered Nurse License.

Experience:
  • A minimum of 7 years of relevant experience.

Knowledge, Skills, and Abilities:
Proficient understanding of general disease management processes, bio-psychosocial models, and clinical pathways. Strong interpersonal, written, and verbal communication skills. Ability to effectively organize and prioritize tasks. Proficient in utilizing computer and software applications.

Key Responsibilities:
Facilitates patient discharges as per physician orders.

Engages in consultation and collaboration with hospitalists, physicians, RN case managers, social workers, educators, and other healthcare professionals, as well as family members, to ensure effective and timely patient discharge.

Ensures all discharge protocols are executed appropriately. Assists with admissions as directed. Acts as a communication link for the physician, interacting with all relevant healthcare personnel, family, and patients. Collaborates with hospitalists to discuss and enhance patient care. Maintains inventory and supplies in the sample closet as required.

Decision-Making Authority:
Exercises independent judgment in planning operational sequences and making minor decisions within a complex technical or professional context.

Working Relationships:
Coordinates the activities of others without direct supervision. Leads peers in similar tasks without supervisory responsibilities. Engages directly with patients and/or customers. Interacts with internal customers through various communication methods. Collaborates with external customers and healthcare professionals, frequently liaising with individuals at the Director level or higher.
