Position Overview:
The Clinical Care Coordinator for Hospitalists plays a vital role in assessing, planning, implementing, and evaluating nursing care as guided by the Hospitalist Program, aimed at enhancing the efficiency of hospitalist services.

Educational Requirements:
Completion of a recognized nursing program approved by a state board of nursing, leading to a diploma or degree.

Licensure and Certification:
Possession of a valid multi-state or Oklahoma Registered Nurse License.

Experience:
  • A minimum of 7 years of relevant experience in a healthcare setting.

Knowledge, Skills, and Abilities:
Proficient understanding of disease management processes, bio-psychosocial models, and clinical pathways. Strong interpersonal, written, and verbal communication skills. Ability to effectively organize and prioritize tasks. Proficient in utilizing computer systems and software.

Key Responsibilities:
Facilitates patient discharges as directed by the physician.

Collaborates with hospitalists, physicians, RN case managers, social workers, educators, and other healthcare professionals, as well as family members, to ensure effective and timely patient discharge.

Ensures that all discharge procedures are executed appropriately. Assists with patient admissions as required. Acts as a communication link between the physician and relevant healthcare personnel, patients, and families. Engages with hospitalists to discuss and coordinate patient care. Manages the inventory of medical supplies and ensures necessary stock levels are maintained.

Decision-Making Authority:
Utilizes independent judgment to plan operations and make minor decisions within a complex professional environment.

Collaborative Relationships:
Coordinates activities among team members (does not supervise). Collaborates with patients and/or customers directly. Engages with internal and external customers through various communication methods. Works closely with other healthcare professionals and staff, often interacting with individuals at the Director level or higher.
