Compensation: $84,000 - $130,000

Role Summary:

The role of Case Management involves a collaborative approach that includes patients, caregivers, nursing staff, social workers, physicians, payers, support personnel, and the broader community.

This process enhances communication and care coordination throughout the healthcare continuum, ensuring effective transitional care management.

Essential Qualifications:

  1. Master's Degree in Social Work from an accredited institution.
  2. Possession of a California Clinical Social Worker License upon hire.
  3. A minimum of 2 years of experience in a healthcare-related environment (Acute, Ambulatory, or Post-Acute).
  4. At least 2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
  5. Proven expertise in discharge planning.
  6. Current experience in Inpatient Social Work.
