About the Role

We are seeking a highly motivated and experienced Care Coordinator to join our team at AltaPointe Health. As a Care Coordinator, you will play a critical role in providing HIV prevention and outreach services to our community.

Key Responsibilities
  • Liaise between HIV Prevention and Outreach/Community Agencies and Programs
  • Provide community outreach and HIV/HEPC prevention education
  • Provide HIV and HEPC testing clinics identified at risk populations within targeted communities
  • Administer oral swab test kits within community outreach clinics, and properly dispose of medical waste according to Accordia Infectious Control Policy
  • Provides ongoing monitoring and care coordination for patients referred to chronic care and PrEP services
  • Assists with meeting reporting requirements of the HRSA and UAB grants
  • Provides support and advocates on behalf of patients, to secure community resources and needed services
  • Effectively collaborates and consults with staff members, including clinicians and pharmacists, and outside sources in the delivery and arrangement of HIV/HEPC Prevention Services to targeted populations
  • Provides education on virus management and healthy living skills within educational outreach groups
  • Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
  • Provides appropriate linkage to chronic care (Ryan White Centers) and PrEP services within community
  • Effectively demonstrates the application of HIV/HEPC Prevention and Outreach specific competencies

  • Attends appropriate in-services training and other workshops
Qualifications
  • Bachelor's degree in a behavioral health, social services, or public health related discipline preferred
  • A certification of case management training by Alabama State Department of Mental Health and experience within the Care Management field preferred or work-related experience
  • Valid verifiable state-issued driver's license
  • Knowledge in high-risk care specialties, outlined below, to be attained within six (6) months of employment:
    • Knowledge of chronic medical conditions
    • Basic knowledge of growth and development of child consumers as related to all intellectual, emotional, spiritual, and physical aspects consumer development
    • Knowledge of cultural diversity
    • Knowledge of Trauma Informed Care principles
    • Knowledge of behavior management, crisis intervention, and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practices
    • Knowledge of HIV education and prevention methods
    • Knowledge of HIV and HEP C viruses and PrEP/PEP
    • Knowledge of social determinants of health
    • Knowledge of available referral sources for physical health, mental health, and social services
    • Basic knowledge of utilizing Electronic Health Records
    • Basic knowledge of DMH and Medicaid standards
