Responsibilities:

As an RN, your primary responsibilities will involve coordinating patient care with the interdisciplinary team, which includes physicians, patients/families, and referring agencies.

You will provide skilled nursing care for patients of all ages in their residences, taking on the responsibility for care coordination.


Key Functions of the RN:
  • Manage cases and offer clinical guidance to the interdisciplinary team, physicians, and families.
  • Develop, implement, and document the patient care plan.
  • Deliver care while adhering to infection control protocols to safeguard both staff and patients (OSHA standards).
  • Ensure continuity of high-quality patient care with appropriate documentation.
  • Monitor assigned cases to confirm compliance with third-party payer requirements.
  • Exhibit commitment to professional growth and competency.
  • Promote the agency's philosophy and administrative policies.
  • Perform on-call duties and provide on-call support to patients and families as required.
  • Supervise and guide home health aides and LPNs.

Qualifications:
Education:

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.


Licensure:

Current State license as a Registered Nurse and a valid Driver's License.


Experience:

A minimum of two years of experience as a Registered Nurse in a clinical care environment, with a preference for home health or hospice experience.


Skills:

Demonstrated nursing skills as per generally accepted standards of practice, along with strong interpersonal abilities. Proof of CPR certification, Hepatitis consent/declination, and Covid Vaccination or eligibility for a religious/medical exemption is required.


Transportation:

Reliable transportation and valid auto liability insurance are necessary.
