The Social Services Coordinator will ensure that these core values are consistently upheld by the team.

QUALIFICATIONS:
To be successful in this role, candidates must meet the following requirements:
  • High school diploma or relevant qualification.
  • Must hold a minimum of a Licensed Bachelor Social Worker (LBSW) from an accredited institution.
  • Possession of a valid driver's license or reliable transportation.
  • Preferred: One year of experience as a social worker in a skilled rehabilitation, skilled nursing, or Long-Term Care (LTC) setting.
  • Experience with electronic medical records is a plus.

CERTIFICATES, LICENSES, REGISTRATIONS:
Must possess a current and active LBSW license to practice as a social worker in the state.
