  • Coordinate with healthcare teams to ensure timely and effective transitions of care, including hospital discharges and post-acute care
  • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and progress
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

  • Graduation from an accredited school of nursing and a current Alabama RN license or current Multi-State RN License in accordance with the Nurse Licensure Compact (NLC) for Alabama
  • Three years of professional nursing experience in the area of clinical focus
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations
  • Strong analytical and problem-solving skills, with ability to think critically and make sound judgments
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
