Responsibilities:


The Patient Navigation Coordinator offers personalized support to patients, families, and caregivers to ensure patient-centered, evidence-based care. This role acts as a vital link between patients and healthcare providers, facilitating clinical services and empowering patients to actively participate in their care. The coordinator collaborates with patients to navigate challenges in disease management by leveraging all available resources, both internal and external, to enhance care outcomes.

The coordinator serves as a guide for patients and their families throughout the healthcare journey, maintaining regular communication through various channels, including in-person meetings, phone calls, written correspondence, and emails. They ensure timely communication with the patient's care team and primary care provider to swiftly identify and address patient issues, thereby preventing delays in care, unnecessary emergency room visits, and hospitalizations.

Additionally, the coordinator stays informed about all aspects of service line care and treatment methodologies, providing patients and their families with information regarding access to resources and community support. They remain updated on available resources from pharmaceutical companies, charitable organizations, and community and faith-based groups.

The role includes maintaining navigation records promptly using a database while upholding confidentiality and demonstrating professionalism in interactions with patients, families, and healthcare professionals. Responsibilities also involve organizing patient schedules and managing appointments to ensure timely services, facilitating the scheduling of clinical consultations and support services involving the medical care team.

The coordinator follows patients throughout the care continuum, including inpatient admissions, and collaborates with inpatient care management resources. They match unmet needs with referrals to internal and external resources, such as social workers, dietitians, financial counselors, and community partner agencies.

Providing emotional and educational support for patients and families is essential, as is identifying alternative payment resources for uninsured and underinsured individuals and assisting with enrollment. The coordinator acts as a patient advocate within the multidisciplinary team, ensuring smooth transitions for patients from one phase of care to the next, promoting self-management and independence in self-care.

Furthermore, the coordinator articulates service and customer relations improvements to enhance patient retention and participates in service line performance improvement initiatives. Regular and prompt attendance is required, along with the ability to work as scheduled and fulfill overtime requirements as necessary.


Qualifications:


A high school diploma or equivalent is required, along with two years of relevant experience; alternatively, successful completion of a vocational program in a healthcare-related field with one year of related experience is acceptable. Higher education may substitute for the required experience on a year-for-year basis.
