Key Responsibilities

The Home Health Patient Care Manager is accountable for overseeing and coordinating clinical services, ensuring the delivery of nursing care to patients in their residences as directed by their healthcare providers and in accordance with relevant laws, regulations, and organizational policies.
  • Delivers clinical services within the defined scope of practice, adhering to the care plan and collaborating with other healthcare professionals.
  • Handles referrals, assigns appropriate clinicians, evaluates patient orders, and schedules initial care visits.
  • Coordinates the assessment of patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Continuously assesses patient needs, ensuring that care is tailored to individual requirements, appropriate, and compliant with physician directives.
  • Reviews assessments and care plans regularly, consulting with clinicians for recommendations as necessary.
Qualifications

Education & Experience Requirements:
  • Valid RN licensure in the state of practice
  • Current CPR certification is mandatory
  • Possession of a valid Driver's License, vehicle insurance, and access to reliable transportation
  • For California: Minimum of one year of professional nursing experience.
  • For Louisiana: At least one year of clinical experience with unrestricted RN licensure.
