Reports to: SSVF Navigation Team Lead
Position SummaryThe SSVF Navigator plays a crucial role in connecting eligible Veterans to the Supportive Services for Veteran Families (SSVF) program, as well as other VA, mainstream, and community resources. This position involves assessing Veteran families to identify the most suitable interventions for homelessness and providing necessary referrals. The Navigator will collaborate with the SSVF Eligibility Screening Specialist during the intake process until the assignment with an SSVF case manager is made. Additionally, the Navigator will ensure that Veterans who do not qualify for SSVF are directed to alternative services and resources.
Key Responsibilities- Establishes rapport with Veteran families facing homelessness or at high risk of becoming homeless.
- Evaluates Veteran families for eligibility in the program.
- Facilitates appropriate referrals to the SSVF program and other community resources.
- Coordinates emergency services for clients who are literally homeless.
- Prepares clients for the intake process and provides assistance throughout.
- Helps clients obtain necessary eligibility documentation, which may include providing transportation.
- Ensures all eligible Veterans are assigned a case manager within three business days of identification.
- Maintains detailed outreach and engagement records in the Homeless Management Information System (HMIS) and other databases for reporting purposes.
- Enters data into various systems promptly, adhering to HMIS data entry standards.
- Transports clients as needed for housing searches and access to community resources such as food banks and employment services.
- Collaborates with the Systems team to identify clients in greatest need and assists in linking them with resources, including housing.
- Works a flexible schedule to accommodate the needs of clients and the agency.
- Welcomes visitors, responds to inquiries, and maintains security protocols.
- Participates as a team member in maintaining operations and fulfilling the organization's mission.
- Engages in professional development activities, including training and research.
- Performs other duties as assigned.
- Associate's degree in a relevant field or equivalent work experience.
- Demonstrated commitment to addressing homelessness.
- Successful completion of a Level II background check.
- Must pass a federal-level drug screening; possession of a medical marijuana card is not acceptable under federal law.
- A valid driver's license and a clean driving history for the past three years.
- Must obtain required vehicle insurance within 30 days of onboarding.
- Excellent interpersonal and communication skills, both written and verbal.
- Ability to organize resources and prioritize tasks effectively.
- Familiarity with community resources.
- Proficient in building trust and rapport with individuals from diverse backgrounds.
- Ability to work independently and as part of a small team.
- Knowledge of federal, state, and community funding sources.
- Strong public service orientation.
- Ability to foster a collaborative work environment.
- Flexibility and adaptability in a dynamic setting.
- Proficient in using personal computers, including Microsoft Office, and capable of learning data entry systems.
- Ability to maintain confidentiality of client information.
- Ability to walk, stand, or sit for extended periods in an office or field environment.
- Occasional lifting or moving of items up to 15 pounds.
- Capability to operate a motor vehicle for local travel.
