Reporting To: SSVF Housing Navigation Team Lead

Position Summary:

The SSVF Housing Navigator will be stationed in the Jacksonville Office and will serve the surrounding counties. This role will collaborate closely with the SSVF Program and the Gainesville Veteran Affairs Medical Clinic.

To effectively combat Veteran homelessness in the Jacksonville area, this position will implement innovative strategies and solutions. The primary responsibility of this role is to facilitate the swift housing placement of Veterans facing homelessness who are enrolled in both the SSVF and HUD-VASH Programs. The Housing Navigator will operate as both a case manager and housing expert, assisting clients in evaluating their housing needs and preferences while connecting them with suitable housing providers.

Key Responsibilities:

  • Collaborate with clients, SSVF Case Managers, and/or VASH Social Workers to comprehensively assess Veterans' housing needs and preferences.
  • Maintain regular communication with Veterans to ensure their active participation in the housing process.
  • Transport clients to various housing opportunities as needed.
  • Assist Veterans in the application process and in gathering necessary documentation for housing.
  • Proactively identify new housing options for Veterans experiencing homelessness.
  • Establish and nurture relationships with landlords and property managers through consistent engagement.
  • Ensure accurate entry of vendor and unit-specific information into relevant systems.
  • Provide updates to stakeholder groups, including SSVF meetings and Continuum of Care committees.
  • Stay informed about landlord/tenant laws and regulations.
  • Address tenant/landlord disputes and facilitate resolutions.
  • Act as a liaison among landlords, clients, and SSVF Case Managers/VASH Social Workers as necessary.
  • Commit to the Housing First approach, treating all individuals with dignity and respect.
  • Document all case-related activities in the Homeless Management Information System (HMIS) and other databases.
  • Perform additional duties as assigned.

Qualifications:

  • Associate's degree in a relevant field or equivalent professional experience.
  • Must pass a Level II background check.
  • Clear a federal-level drug screening; possession of a medical marijuana card is not acceptable under federal law.
  • Valid driver's license with a clean driving record for the past three years.
  • Must obtain required vehicle insurance within 30 days of onboarding.

  • Ability to organize resources and prioritize tasks effectively.
  • Familiarity with community resources.
  • Proficient in building trust and rapport with individuals from diverse backgrounds.

  • Knowledge of federal, state, and community funding sources.
  • Strong public service orientation.
  • Ability to foster a collaborative work environment.
  • Flexibility and adaptability in a dynamic setting.
  • Proficient in Microsoft Office and data entry into management systems.
  • Ability to maintain confidentiality of client information.

Physical Requirements:

  • Ability to walk, stand, or sit for extended periods in various environments.
  • Occasional lifting or moving of items up to 15 pounds.
  • Capability to operate a motor vehicle for local travel.
