At Independence Care System, we are seeking a highly skilled Care Navigator to join our Health Home team. As a Care Navigator, you will play a critical role in coordinating members' overall care, ensuring they receive the necessary health and social supports to maintain their health, mobility, and independence in the community.

Key Responsibilities:

  • Coordinate access to all needed services and supports for members, including facilitating communication and providing coordination of care between health and community providers.
  • Develop and implement members' Plans of Care (POCs), including creating SMART member-stated wellness and recovery goals, target timeframes, strategies, actions, and paid/unpaid supports involved.
  • Monitor and evaluate members' health and wellness needs, including preventative and specialty care, through the creation of a disability-competent member-centered plan of care.
  • Collaborate with members, the Nurse Care Manager, and other departments to ensure proper handling of all member requests, inquiries, and concerns.

Requirements:

  • Bachelor's Degree and at least 1 year of full-time, relevant work experience.
  • Demonstrated problem-solving skills.
  • Customer Service experience in a fast-paced environment or experience in a healthcare facility.
  • Proficient in MS Word, Excel, and Outlook.
  • Ability to work and converse efficiently with all levels of colleagues, clients, and other external contacts.
