Job Summary

We are seeking a highly skilled and dedicated Service Coordinator to join our team at the Helen Farabee Center. As a Service Coordinator, you will play a critical role in providing services to eligible individuals with Intellectual Disabilities (IDD) to gain access to medical, social, educational, and other needed services.

Key Responsibilities
  • Provide services to eligible individuals with IDD, including crisis prevention and management, screening and assessment, monitoring, consulting, and advocacy.
  • Develop and implement Person Directed Plans to ensure individuals receive the necessary services and supports.
  • Collaborate with schools, service providers, and community resource agencies to ensure individuals receive comprehensive services.
  • Monitor and evaluate services and supports for individuals enrolled in programs, including tracking progress and reporting to stakeholders.
  • Advocate for individuals with IDD, including attending Admission, Review, and Dismissal (ARD) meetings and working with schools to ensure individuals receive special education services.
Requirements
  • Bachelor's or advanced degree from an accredited college or university in a social, behavioral, or human service field.
  • Current driver's license and insurability under Center insurance.
  • Ability to communicate effectively orally and in writing, with strong interpersonal skills and customer service orientation.
  • Knowledge of Intellectual Disabilities service delivery system and ability to relate verbal instructions and communicate in a courteous and effective fashion.
  • Extensive knowledge of LIDDA principles and ability to demonstrate good documentation and organizational skills.
  • Skilled in the use of personal and network computers, word processing, database, spreadsheet, and office management software.
Work Environment

This position requires frequent travel to individual's homes, schools, and Day Habilitation sites, with the ability to work flexible hours and schedule. The Helen Farabee Center incorporates principles of trauma-informed care, including cultural sensitivity, in all areas of service delivery.

Reassignment is a condition of employment that may occur at the employer's discretion. Your work location and hours could change based on program needs.
