The Clinical Nurse Specialist will act as a key support to the healthcare provider, facilitating the ongoing healthcare requirements aimed at optimizing health for various chronic conditions, including diabetes, hypertension, and hyperlipidemia. This role involves initiating care and placing orders for designated patients following established protocols to achieve optimal health outcomes. The Clinical Nurse Specialist will be responsible for managing all scheduled patients for all providers within a designated office, ensuring seamless communication with all healthcare professionals to deliver quality, patient-centered care.
Key Responsibilities:
- Collaborate effectively as a vital member of the clinical team to provide patient-focused care.
- Review and highlight all scheduled patient records using electronic alerts to identify areas requiring attention during upcoming visits.
- Recognize clinical opportunities for necessary follow-up care.
- Order diagnostic tests as needed, adhering to the established protocols set by nursing management and providers.
- Manage variables from the established protocols, which may include:
a. Fall Risk Assessment
b. Smoking Cessation
c. BMI (Body Mass Index)
d. Mammogram
e. Bone Density
f. Pneumovax (Pneumonia vaccine)
g. Fecal Occult
h. Diabetic measures
- Maintain communication with providers and other healthcare staff as necessary to assist with various tasks in the office.
- Perform additional duties as assigned.
Qualifications:
- Minimum High School diploma or equivalent required.
- LPN licensed in the state of employment or Registered/Certified Medical Assistant.
- 3-5 years of nursing experience and proficiency in utilizing an EMR system.
Skills and Knowledge:
- Ability to work autonomously while strictly adhering to protocols.
- Detail-oriented with excellent written and verbal communication skills.
- Strong time-management, organizational, and problem-solving abilities.
Supervisory Responsibilities:
- None
