Key Responsibilities:

  • Provide primary care services to patients in skilled nursing facilities, including long-term care, short-term rehab, memory care, and clinically complex patients.
  • Drive high-quality, safe, and cost-effective care of patients in our local post-acute care communities.
  • Conduct comprehensive patient assessments, develop and implement individualized care plans, and provide ongoing monitoring and evaluation of patient progress.
  • Collaborate with interdisciplinary teams, including hospice, palliative care, and home care agencies, to ensure seamless transitions of care.
  • Document patient information accurately and timely in our electronic medical record system.
  • Provide leadership, guidance, and mentorship to our clinicians within the facilities, promoting a culture of excellence and continuous improvement.
  • Participate in clinical case reviews, share best practices, and develop tools and guidelines to enhance the quality of care.
  • Collaborate with leadership to review care coordination initiatives and develop collaborative intervention plans.

Requirements:

  • Board certification in internal medicine, family medicine, emergency medicine, or a related field.
  • Minimum 2 years of experience in primary care, with a focus on post-acute care.
  • Strong leadership and communication skills, with the ability to work effectively with interdisciplinary teams.
  • Proficiency in electronic medical record systems and other relevant software applications.
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
