Job Summary

As a Central Intake Coordinator at Trilogy Health Services, you will play a vital role in ensuring seamless patient access to our senior living facilities. Your primary responsibility will be to perform clinical and financial reviews of referrals from hospitals, verifying insurance benefits and obtaining prior authorizations as needed. You will also be responsible for approving or escalating referrals based on the clinical or financial capabilities of each campus.

Key Responsibilities
  • Monitor hospital portals for new referrals and confirm receipt in the portal.
  • Coordinate the initial intake and clinical assessment of each referral, working to green, yellow, or red-light status.
  • Communicate with hospital liaisons to follow up on referrals.
  • Send all received referrals for benefit verification.
  • Document all received referrals in the Customer Relationship Management software.
  • Utilize portals for documentation of status of each referral (accepted, denied, considering).
  • Evaluate referrals to be assigned to the appropriate campuses based on payer source and bed availability.
  • Support sales throughout the initial referral process.
Requirements
  • Associate Degree
  • 3-5 years of experience
  • Ability to lift at least 50lbs
  • Excellent verbal and written communication skills
