The Community Resource Specialist is responsible for linking Wraparound Facilitators to available community programs, Coordinated System of Care (CSoC) service providers, and other supportive services. This position will establish and maintain relationships with referral partners and other resources in the community to ensure that participants are provided with an array of individualized services and supports. In addition, this role will be responsible for tracking service utilization, assessing gaps in services, and working with the SMO to address these service gaps. The CRS will assist the teams in finding resources to meet their individual needs. If necessary, the CRS will work with the teams to develop a service that may not be readily available in the community. The CRS will track referrals for CSoC services and assist with developing new service providers for the network. The incumbent will assist all partner agencies (such as mental health, special education, child welfare, developmental disability, and juvenile justice) in following the mission, principles, and values of Wraparound. By assisting these agents effectively, families, care coordinators, and child and family teams, each client-family enrolled in Choices projects will experience positive clinical and fiscal outcomes.

Essential Duties and Responsibilities

  • Establish and maintain referral partnerships with various community organizations and service providers.
  • Develop and maintain a list of community resources.
  • Track referrals and service utilization of all CSoC youth.
  • Understand CSoC waiver requirements to ensure compliance with service provisions.
  • Increase the community’s awareness of Wraparound Services.
  • Assist the Executive Director with the establishment and maintenance of a functioning Community Team.
  • Works closely with finance team to ensure authorization issues and other care coordination fiscal matters are expediently resolved.
  • Assures that providers thoroughly understand and practice:
    • family driven and strengths-based care,
    • outcomes based service and support, and
    • cultural competence/diversity.
  • Encourages and trains providers to model Choices’ values and wraparound principles.
  • Ensures that service authorizations are completed on time and accurately entered into TCC/POC.
  • Provides training and expertise to internal staff, contract staff and/or providers, and stakeholders, to ensure that staffing, billing, and interagency processes are working effectively.
  • Provides support and expertise to internal and/or contract staff in proper service authorizing.
  • Facilitates regular provider meetings and through training, coaching, and modeling; supports providers and internal/contract staff in understanding all issues related to authorizing, contracting, and invoicing for services.
  • Actively participates in internal and external training opportunities.
  • Exhibits enthusiasm for learning and personal growth.
  • Understands and can describe key components of all Choices’ programs/lines of service.
  • Ensures that strength-based, family-driven, values-based practices are understood and practiced by support providers with clients, the system, the staff, and the organization.
  • Establishes relationships with existing community programs and resources to help them develop capacity and skills to serve the unique needs of Choices’ families.
  • Works closely with local family support organization to ensure participation in the system of care.
  • Promotes community ownership of the system of care.
  • Exhibit a positive attitude and ability to engage others towards a common goal.
  • Represents Choices’ Values and follows Choices’ Guiding Principles within the community at all times.

Qualifications
  • Minimum of bachelor’s degree; master’s degree preferred. Significant experience related directly to position duties may be considered as substitute for formal education.
  • Minimum of two years of supervisory experience.
  • Experience in customer-oriented sales or customer service a plus.
  • Demonstrated ability to develop and monitor service delivery contracts and to address issues related to accounts payable and receivable.
  • Demonstrated skill in fiscal management activities, team building, and development.
  • Strong knowledge of and genuine respect for youth and adults with mental health issues and a firm commitment to empowering their families.
  • Strong communication and writing skills. Bilingual skills (especially Spanish) a plus.
  • Highly organized and detail oriented.
  • A valid driver’s license in his/her state of residence and reliable transportation.
  • Demonstrated ability to:
    • Work effectively with internal and external individuals, including other professionals in the community.
    • Work effectively as a member of a team.
    • Effectively communicate to various internal and external audiences in both person and through various electronic media.
    • Manage time and work effectively with minimal supervision.
    • Effectively manage multiple priorities simultaneously.
