County Government Agency seeks a Case Manager Supervisor for the Mental Health Forensic Department. Case Manager responsibilities include all aspects of case management including, referral and placement of consumers, file maintenance, participant contacts and case notes, data entry and reporting requirements, development and maintenance of service plans and the provision of supportive services. Strong organizational, creative and interpersonal skills required. Must be flexible and solutions oriented. Ability to motivate and advocate for participants is essential. Bachelor's degree in a social services or related field is required. Must have clean driving record, PA Child Abuse History and Criminal History Record clearances. Flexible hours are required.) Position Description Position is that of DS Support Coordinator Supervisor for the Developmental Services Program. Oversees and supervises Supports Coordinators 1 & 2, and Social Service Aides. Supervises the development of DS Service Plans as they affect outcomes of goals and waiver budgeting. Oversee HCSIS training of Caseworkers 1 & 2 and Social Service Aides focusing on monitorings, plans, incident reports and case notes to insure Self-Determination. Assures compliance with AE oversight standards for support coordination. Position Requirements To perform the DS Casework Supervisor job successfully, an individual must be able to perform each essential responsibility satisfactorily. These requirements are representative, but not all-inclusive, of the knowledge, skill, and ability required. Excellent computer skills, including Word, Excel and Outlook in a Microsoft Windows environment. Effective oral and written communication skills. Excellent interpersonal skills. Able to exhibit a high level of confidentiality. Excellent organizational skills. Must be able to identify and resolve problems in a timely manner. Must be able to gather and analyze information skillfully. Education and Experience Two years of professional experience in public or private social work and a bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; OR Any equivalent combination of experience and education which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences. Physical Demands While performing the responsibilities of the DS Casework Supervisor, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers, to handle or feel and to manipulate keys on a keyboard. The employee may be required to stand, walk, reach with arms and hands, climb or balance, stoop, kneel, crouch or crawl and, enter and exit a vehicle. The employee may need to work with arms above shoulder height, bend at the waist or work bent at the waist, lift or carry items or occasionally work with an uncommon level of noise. Vision abilities required by this job include close vision. Work Environment Normal office environment. The noise level in the work environment is usually quiet to moderate. Position may require travel throughout Carbon, Monroe and Pike counties in addition to periodic travel to othe locations in Pennsylvania. Agency vehicle is usually available during work hours. Position Duties: Train and supervise assigned case managers in Carbon, Monroe, and Pike Counties in their responsibilities in assuring appropriate consumer service through individual and group staff meetings, monitoring case activity and reviewing records. Staff procedures stipulated for each case manager function; are familiar with philosophy, policies and procedures; all records are in order; reviews are done in systematic fashion; staff use automated processes. Schedule staff for ongoing training. Promote coordination of services with other agency and community services. Attend meetings, such as CSP, CASSP and other interagency meetings, to promote coordination and collaboration of needed services, develop policies and procedures as needed. Help resolve consumer and system issues. Develop and implement policies and procedures for the delivery of mental health case management in conjunction with the Director of Case Management in keeping with overall agency philosophy and priorities. Write policies and procedures related to referrals, prioritization of cases, intake, assessment, service plan development, and delivery of service. Provide regular (at least monthly) reports to the Director of Case Management on all components of the program. Assure availability of mental health services to consumers including traditional and alternative MH Services, FSS, CRR, Housing resources, Adult and children treatment/services, respite, correctional facilities and assist with planning and development related to these. Visit programs and provide technical assistance as needed. All reports, budget data, planning data, and projections submitted on or before due dates, in approved formats. Assure that case management activities meet or exceed regulations and agency standards by disseminating data, monitoring performance, and evaluating staff and providers; maintain cooperative relationship with clients, providers, families, advocacy groups, and the community; participate in regular meetings, staff meetings, provider/community meetings. Develop evaluation processes, meeting with consumer group, monitor allocations, expenditures, and satisfaction with services. Organize, provide and participate in consultation, education and training for staff, other organizations, families, consumers and the community; participate in regular meetings, staff meetings, provider/community meetings.
