Nurse Practitioner Responsibilities:- Collaborate with physicians and palliative care team
- Manage advanced illness from assessment through discharge
- Establish individualized care plans for patients
- Facilitate obtaining necessary care for patients
- Prescribe medications and order diagnostic tests as needed
- Maintain effective working relationships with care team and others
- Communicate and document clinical findings in patient's medical record
- Excellent leadership, communication, and decision-making skills
- Working knowledge of palliative medicine and healthcare regulations
- Masters of Science in Nursing and minimum of two years Nurse Practitioner experience
- Current Nurse Practitioner certification/license and state licenses as required
