Nurse Practitioner Responsibilities:
  • Collaborate with physicians and palliative care team
  • Manage advanced illness from assessment through discharge
  • Establish individualized care plans for patients
  • Facilitate obtaining necessary care for patients
  • Prescribe medications and order diagnostic tests as needed
  • Maintain effective working relationships with care team and others
  • Communicate and document clinical findings in patient's medical record
Nurse Practitioner Requirements:
  • Excellent leadership, communication, and decision-making skills
  • Working knowledge of palliative medicine and healthcare regulations
  • Masters of Science in Nursing and minimum of two years Nurse Practitioner experience
  • Current Nurse Practitioner certification/license and state licenses as required
