Qualifications for Social Work:

  • Licensed Social Worker
  • Master's or Bachelor's degree from a program accredited by the Council of Social Work Education
  • A minimum of 2 years of experience in a medical environment, with exposure to hospice and/or palliative care
  • Capability to work across both hospice and palliative care programs
  • Provide emotional, social, and grief support to patients and their families, in collaboration with spiritual and bereavement support staff.
  • Engage in Interdisciplinary Group meetings, identifying social-psychological issues and their connections to medical conditions.
  • Assist in the coordination and execution of Ultimate Gift Programs, including Gift of a Day and Veteran Recognitions.
  • Possess a reliable personal vehicle, valid driver's license, and the ability to safely operate the vehicle for patient visits and job responsibilities.
