We are currently seeking a skilled Palliative Care Nurse Practitioner to enhance our healthcare team. This role emphasizes delivering exceptional patient care within a collaborative environment.


Compensation: The salary for this position ranges from $100k to $120k annually, reflecting the candidate's education and relevant experience. Position Summary:

The Palliative Care Nurse Practitioner will support the Medical Director in overseeing and implementing the medical aspects of hospice and palliative services. This role involves providing clinical guidance and education to a multidisciplinary team, collaborating with both internal and external healthcare providers to improve access to services. The Nurse Practitioner will participate in the development of the agency's Palliative Care program and will work within an interdisciplinary team to assist patients with serious illnesses in managing symptoms, clarifying care goals, addressing end-of-life concerns, and planning transitions.

Key Responsibilities:

  • Diagnose and manage acute, episodic, or chronic health conditions.
  • Prescribe necessary medications and treatments, including controlled substances, in accordance with delegation and licensing regulations, following collaboration with attending physicians.
  • Conduct thorough physical examinations of patients.
  • Develop and implement policies and procedures for patient management.
  • Facilitate referrals to other healthcare professionals and facilities.
  • Communicate effectively with collaborating physicians or specialists regarding patient care.
  • Provide hospice care through comprehensive evaluations, health promotion, diagnosis, and management of health issues, while coordinating with patients, families, and healthcare practitioners.
  • Certify and re-certify patient eligibility for the Hospice Program in collaboration with attending physicians.
  • Offer clinical support to the hospice multidisciplinary team.
  • Consult with primary care physicians or specialists as necessary regarding patient medical conditions.
  • Act as a liaison between attending physicians and the hospice team, ensuring continuous communication about patient care plans.
  • Assess patients' medical statuses in various settings based on a scheduled visit plan.
  • Collaborate with the hospice provider team to ensure continuous care delivery.
  • Participate in Patient Care Conferences as needed.
  • Provide ongoing education to the community and healthcare professionals regarding hospice services.
  • Assist in designing and developing a home-based palliative care program.
  • Supervise care for palliative patients in home-based programs, serving consultatively to attending physicians.

Qualifications:

  • Master's degree and graduation from a New York State approved Nurse Practitioner program. Current licensure and certification as a Nurse Practitioner by a nationally recognized certifying organization in a relevant specialty area.
  • Completion of pharmacotherapeutic requirements for independent prescriptive privileges.
  • Possession of a National Provider Identifier (NPI) and Federal Drug Enforcement Administration (DEA) Registration Certificate for prescribing controlled substances.
  • Prior experience in nursing or related medical fields.
  • Strong rapport-building skills with patients.
  • Able to thrive in a dynamic, fast-paced environment.
  • Excellent written and verbal communication abilities.
  • Demonstrated leadership qualities with at least five years of experience, including pain and symptom management.
  • Proof of completed COVID-19 vaccination.
