About the Role
Hartford Healthcare (HHC) is seeking a skilled Palliative Care Advanced Practitioner to join their interdisciplinary care team. As a key member of the team, you will work collaboratively with attending physicians and other healthcare professionals to deliver expert palliative care services to patients and their families.
Key Responsibilities
- Develop and implement individualized care plans to optimize patient function and quality of life
- Provide expert pain and symptom management, taking into account factors such as fear, communication barriers, economic issues, and cultural traditions
- Collaborate with the care team to coordinate patient care and ensure seamless transitions
- Participate in ongoing education and professional development to stay current with best practices in palliative care
Requirements
- Unrestricted State of Connecticut Medical License (or eligible)
- Training as a Physician Assistant or Nurse Practitioner
- Preferred experience: 1+ years in palliative care
- National Board Certification in Palliative Care
