Key Responsibilities

  • Develop and implement individualized care plans to optimize patient function and quality of life
  • Provide expert pain and symptom management, taking into account factors such as fear, communication barriers, economic issues, and cultural traditions
  • Collaborate with the care team to coordinate patient care and ensure seamless transitions
  • Participate in ongoing education and professional development to stay current with best practices in palliative care

Requirements

  • Unrestricted State of Connecticut Medical License (or eligible)
  • Training as a Physician Assistant or Nurse Practitioner
  • Preferred experience: 1+ years in palliative care
  • National Board Certification in Palliative Care
