The Palliative Care Program Manager is a skilled registered nurse tasked with the oversight of care coordination for both new and ongoing Palliative and Hospice patients. The Palliative Care Program Manager will:
- Collaborate closely with the Hospice Clinical Manager to ensure the quality and coordination of patient care delivery.
- Review new patient referrals, work with the care team to ensure all necessary documentation is complete, and coordinate durable medical equipment (DME), medications, and admission scheduling.
- Assist with daily scheduling and manage visit assignments as needed.
- Facilitate patient care transitions for changes in hospice care levels or program transitions.
- Oversee the management of new pending palliative referrals and maintain the palliative care caseload.
- Act as the primary contact for the Hospice and Palliative care nurse practitioner.
- Support Hospice patient triage processes.
- Manage task lists within the MatrixCare electronic medical record system.
- Work collaboratively with all agency departments to ensure patients receive appropriate services.
- Communicate effectively with palliative and hospice patients and their families.
- Serve as a resource for field staff as needed.
- Stay informed about current Hospice and Palliative care policies and practices.
- Participate in interdisciplinary team meetings as required.
- Engage in respectful and professional communication.
- Adhere to agency-wide policies and procedures.
- Perform other duties as assigned.
To be considered for the Palliative Care Program Manager position, candidates should possess:
- An active Registered Nurse (RN) license in New Hampshire.
- A minimum of three to five years of healthcare experience.
- Strong communication and coordination skills.
- Preferred experience in home health or hospice settings.
- Automobile insurance coverage.
- A physical examination completed within the last year.
- A two-step TB test.
- Proof of influenza vaccination.
