Job Summary:

The Swan Hospice is seeking a highly skilled and compassionate Hospice Nurse Practitioner to join our interdisciplinary team. As a key member of our staff, you will be responsible for providing high-quality medical care and palliative services to our patients and their families.

Responsibilities:

I. Re-certification of Patients for Hospice Care

• Conduct thorough medical examinations and document findings in patient clinical records to verify continued medical eligibility for hospice services.

• Perform and document face-to-face encounters and recertification encounters within the required timeframe.

II. Intervention/Patient Care Coordination

• Provide medical examinations and patient home visits as needed to address palliative and end-of-life care issues.

• Collaborate with the attending physician and Hospice Medical Director to manage patient terminal conditions, including prescribing medication and coordinating care.

• Offer consultation and guidance on palliative and end-of-life care issues as needed.

III. Leadership/Professionalism

• Stay current with hospice principles and practices through ongoing education and professional development.

• Foster strong working relationships with hospice staff and other healthcare professionals.

• Utilize critical thinking and problem-solving skills to make informed decisions and recommendations.

• Demonstrate excellent interpersonal and communication skills, both verbally and in writing.

Requirements:

• Current licensure as a Nurse Practitioner in the state of practice.

• Experience in hospice care or a related field.

• Strong knowledge of hospice principles, practices, and regulations.

• Excellent communication and interpersonal skills.
