POSITION PURPOSE:

YouthBuild Case Manager is responsible for providing case management and counseling services to participants enrolled in American YouthWorks' YouthBuild Program.

ESSENTIAL FUNCTIONS AND BASIC DUTIES:

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions and basic tasks. Under direct supervision of the Participant Services Manager, the Case Manager will perform the following functions:

  1. Maintain a caseload of 25-35 YouthBuild participants ages 16-26 providing case management services that include creating individual service plans, crisis intervention counseling, assisting with court/probation related issues, problem solving, home visits, information/community referrals, parent/family support, and assist with end of year follow-up.
  2. Maintain monthly contact with Youthbuild participants that have completed the Youthbuild program and are enrolled in follow up services.
  3. Facilitate and attend restorative meetings with students to address concerns and provide support with their participation in the Youthbuild program.
  4. Have a working knowledge of community partners that can assist YouthBuild participants with social services.
  5. Process and distribute emergency support services to Youthbuild participants
  6. Document all participant services in case notes and appropriate databases
  7. Be able to create an individual service plan for participants and lead new participant orientation activities
  8. Assist with developing and producing informational materials about YouthBuild for community partners
  9. Facilitate Leadership Development/Life skills groups on a weekly basis.
  10. Work with education and vocational teams in providing support in the classroom/on site with members related issues as needed
  11. Attend transition meetings with participants and transition team to assist with wrap around services during participants follow up period.
  12. Write alumni success stories on a monthly basis and contribute to the narrative section of grant reports as needed
  13. Assist in supervising social work interns
  14. Document and report program measures for quarterly DOL reports
  15. Transport members to and from off- site services
  16. Serve as a staff chaperone and driver on Youthbuild field trips and service projects
  17. Attend relevant team meetings and professional development opportunities
  18. Other tasks as assigned by supervisor

QUALIFICATIONS:

EDUCATION / CERTIFICATION: Master's Degree in a related field

Minimum Required

EDUCATION / CERTIFICATION: Bachelor's degree in Counseling, Social Work, Professional License in Social Work, Counseling/Psychology, or Chemical Dependency or a related field.

EXPERIENCE: Four years of previous related experience working with academically and socially at-risk opportunity youth and young adults with criminal records and a variety of lifestyles, behaviors, cultural and spiritual practices. Related experience in jobs training, youth development, working with contracts, reporting measurements, audit request, compliance, member coordination and, or working as a YouthBuild/AmeriCorps model;

EQUIVALENT: Bachelor's degree in the specified or related field of study and 4 years of the specified work experience;
OR High school diploma or GED, and 8- years of work experience such as described in the essential functions and duties of the job description.

PHYSICAL ACTIVITIES:

VEHICLE AND EQUIPMENT INTERACTION: Navigating and/or driving a personal vehicle or 15-passenger company vans; using a computer and computer applications to enter data or process information; Photocopiers and scanners.

WORKING CONDITIONS:

INDOORS: Environmentally controlled conditions, working in an office or shared office setting, working with others in a group or team; contact with others (face-to-face, by telephone, or otherwise), No hazardous conditions. May be required to work in hot, wet, cold, or other significantly unpleasant conditions.

OUTDOORS: Occasional for special events.

ATTENDANCE: Monday - Friday, flexible 40 hour workweek; 1-hour unpaid lunch break; occasionally weekends or after hours to meet deadlines or special projects and other events as needed.

TRAVEL:

REGULAR: Visit service sites and transport participants as needed

OCCASIONAL: Attend professional conferences, workshops, trainings, Home visits; attend court; chaperone day/overnight field trips; annual program retreat; and visit construction sites;
