Registered Nurse Case Manager Compensation

  • The pay for this position is $42 - $46 per Hour
  • Benefits are available to full-time employees after 90 days of employment
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Registered Nurse Case Manager Highlights

  • The required availability for this position is Monday – Friday from 8AM – 5PM

Registered Nurse Case Manager Responsibilities

  • Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care
  • Complete member assessments and all associated documentation, complete contractual outreaches, follow up with any pending member needs
  • Making phone calls to members for contractual outreaches, conducting in home visits with members.
  • Assisting with various needs for member such as DME and waiver needs.
  • Work in community assessing members doing Waiver assessments and placing authorizations

  • Attendance is mandatory for the first 90 days
