Key Responsibilities- Develop and implement individualized care plans in collaboration with the rehabilitation team, ensuring alignment with patient goals and outcomes.
- Coordinate weekly patient care team conferences to facilitate treatment plan development, monitoring, and refinement.
- Communicate effectively with patients, caregivers, and healthcare providers to ensure timely and accurate information exchange.
- Ensure compliance with regulatory requirements, including CMS guidelines, and maintain accurate documentation in Meditech.
- Collaborate with the rehabilitation team to identify and address patient needs, ensuring a comprehensive and coordinated approach to care.
- Current licensure as a Registered Nurse, Social Worker, or Healthcare professional (Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist).
- Certification in Case Management or Rehabilitation Nursing preferred.
- Minimum of 2 years experience in social work or case management in an inpatient setting, with acute/rehabilitation hospital experience preferred.
