Overview

This is a professional position in which the incumbent provides direct care for patients at the Health Care District's Primary Care Clinics. The incumbent works collaboratively with the Psychiatrist and Medical Providers to provide patient assessment and diagnosis, formulate treatment plans, prescribe medications, order laboratory tests, provide patient education, prepare medical records, and refer patients for inpatient care in accordance with statutes, regulations, and protocols regulating the profession. The incumbent works under the supervision of the Psychiatrist and the Medical Director. This position works in acute settings that address crisis mental health and substance use care between crisis settings as well as in an outreach setting with a multidisciplinary team where medical triage may occur for children and adults

The APP (ARPN - Psychiatry or PA-C Physician Assistant) will also be responsible for covering for other providers as appropriate at other clinics during vacations or other absences.

Responsibilities

Provide MedicallyNecessary Triage, Assessment, Treatment, and Follow-up Care

  • Provide behavioral health services within the scope and practice as defined by the Florida Board of Nursing.
  • Conduct triage of basic medical problems, treat basic medical problems, and refer appropriately to the right level of care.
  • Obtain comprehensive health histories, request special diagnostic and laboratory tests, interpret test results, and develop comprehensive psychiatric treatment
  • Provide mental health assessments, diagnoses, and treatment plans in identified areas; provide medication management for patients; and maintain appropriate health records
  • Recommend and execute appropriate behavioral health and substance use referrals, including involuntary or voluntary psych /substance use diagnosis
  • Conduct screening interviews and assessments to stratify safety risks and appropriate levels of care for the patient for mental health and substance use.
  • Prescribe pharmaceuticals therapy consistent with a medical evaluation, treatment plan, and current standards of practice
  • Refer clients for specialized or inpatient interventions as clinical situations dictate
  • Provide medically necessary and appropriate care of acute and chronic psychiatric problems included in the DSM-V,  referring to specialists as appropriate.
  • Perform mental status examinations and appropriate physical exams.
  • Provide phone or telehealth support to mobile response teams if needed.
  • Utilize evidence-based strategies and training to de-escalate patients and their families, while working with the team to keep patients, staff, and bystanders safe.

Collaborate with AppropriateSystems Involved with Patient Care

  • Work cooperatively with the legal system to ensure coordination of services, including court appearances when appropriate.
  • Work collaboratively with community-based partners regarding incoming and outgoing referrals to provide patient-centered coordinated care.

Serve on interdisciplinary Team, Collaborate to Ensure All Patients Receive AppropriateCare

  • Assess, plan, develop, implement, and evaluate an effective and efficient treatment program that will appropriately meet the needs of patients utilizing best care practices, industry standards, organizational guidelines, appropriate accreditation standards, and applicable federal laws, including HIPAA
  • Provide leadership and facilitate a team approach to ensure all patients receive appropriate assessment and care.
  • Triage and prioritize emergency appointments and walk-ins within a structured schedule
  • Participate in a team management approach to patient care/treatment
  • Requires working in a 24x7 model and must be available for night and weekend shifts.
  • Participate in the patients’ plan of care, interdisciplinary team meetings, committee meetings, and rehabilitation meetings when necessary.
  • Discuss findings as needed with the attending physician, consulting staff, and medical service providers. 

 Complete all administrative Tasks/ Functions Appropriate to the Role to Support Operations

  • Maintain appropriate files; develop and follow through on treatment plans within organizational guidelines; and prepare reports on the clinical management of patients
  • Maintain quality through participation in peer review, policy development, and quality improvement programs
  • Maintain appropriate documentation of clinical services, including all necessary billing information
  • Ensure accurate consistent use of encounter forms for all client contacts with behavioral providers to ensure accurate billing of third-party resources
  • Maintain and update professional knowledge and proficiency through continuing education, staff meetings, workshops, and serving on special committees
  • Bill appropriately to ethical standards
  • Other related duties as assigned.

Qualifications
  • Education:  Successful completion of accredited Advanced Practice Registered Nurse (APRN) training according to the Florida Board of Medicine or successful completion of accredited Physician Assistant Studies program (PA-C).
  • Experience:
  • Three (3) to five (5) years previous experience related to the above describe duties. Previous experience assessing and providing medication management for patients with behavioral health disorders in a psychiatric or primary care setting. Experience with children, adolescents, and adults necessary. Working knowledge and experience assessing and treating adults/adolescents with alcohol and other drug abuse/dependence Expert clinical judgment and the ability to be self-directed and demonstrate initiative in an expanded role Advanced Practice Provider.
  • Certification: Must be certified by the ANCC (American Nurses Credentialing Center) as a PMHNP-BC, Current ACLS or BLS certification required.
  • Licensure: Must be licensed as a PMHNP-BC through the Florida Board of Nursing and she must be certified through the American Nurses Credentialing Center (ANCC) · Must have DEA and DEAX number and follow all requirements for the waiver. · Buprenorphine Waiver Training required within six months of hire.
