Position Summary:

The Mobile Crisis Intervention Program Coordinator position will have responsibility for providing direct support and oversight of program and service related duties and support to Mobile Crisis Intervention (MCI) case management and peer certified counseling staff members through modeling appropriate behaviors, training staff members, chart monitoring, and daily schedule adherence, in addition to providing support for direct crisis intervention services with individuals seeking services on a walk-in basis.

The Program Coordinator provides Mobile Outreach follow up support services to individuals experiencing acute emotional crises, with the goal to stabilize the individuals crises in the least restrictive setting possible and support them through follow up services that ensure client is connected or reconnected to any and all available and eligible services. Appropriately utilizes crisis intervention techniques within scope of practice for individual and community stabilization. Program Coordinator duties include but are not limited to providing direction, modeling appropriate behavior, training and guidance of case management and peer certified counseling staff providing services in the community. Assist in monitoring currently enrolled follow-up client charts; inventory and ordering of needed program supplies, inventory and purchasing of program food and meal items, assisting in assessment of program and identifying areas of improvement. This position will partner with Mobile Crisis Intervention Team (MCI) MHPs and Associate Director to implement identified improvements to the program.

Responsibilities:

Direct Service Duties

  • Provide direct services to ensure seeking services on a Walk-in referral basis and/or in need of crisis follow up services
  • Provides documentation for services provided, documents congruently whenever possible and completed all notes by the end of the business days:
  • Appropriately utilizes crisis intervention and de-escalation techniques for community based individual stabilization by accurately assessing risk to self, others or property; implements appropriate clinical decisions based upon intervention and assessment data
  • Provides treatment planning:
  • Assists in developing an appropriate follow up plan of treatment to further support the individuals functioning and recovery after the initial crisis contact either as part of that initial crisis contact appointment or as follow up the following day, week, month, 3 months, or 6 months.
  • Supports recovery concepts by incorporating the individuals and their familys strengths, and needs into the plan components.
  • Establishes and maintains working relationships with community resources
  • Works with hospital liaison, outpatient staff and PACT team members as well as other professionals in the community to coordinate care in relationship to individuals mental health needs.
  • Communicates with internal and external interdisciplinary team members to facilitate good clinical management of individuals with serious mental illness and coordinates treatment with other providers as indicated
  • Facilitates linkages to medical providers or other community resources when appropriate. Works closely with community agencies including law enforcement, Child Protective Services, hospitals, jails and community physicians to coordinate care and arrange for client's treatment, as indicated
  • Responds to requests for services by gathering information from consumers/support persons, recording information, and assessing client needs at the individuals location or in another appropriate location
  • Completes necessary paperwork as required, meeting established timelines and making authorized entries in the medical record, follows all procedures for release of information and maintains complete and accurate, concise records and reports.
  • Fosters teamwork and cooperation between team members and Associate Director and works to understand others on the team and is a cooperative team member who willingly helps others performs related jobs
  • Identifies and prioritizes issues, problems and concerns that need to be addressed and resolved, and facilitates resolution accordingly. Includes others in the decision making process when appropriate to make independent, effective decisions using available information.
  • Maintains confidentiality in accordance with HIPAA regulations and educates individuals on matters requiring informed consent.
  • Participates in supervision with Associate Director- Transformation Health and Crisis Mobile Intervention, multi-disciplinary team staff meetings, training, specialist and psychiatric consultation as indicated/requested and required by the Associate Director- Transformation Health and Crisis Mobile Intervention

House and Client Care Responsibilities

  • Supports daily Operation s of Mobile Crisis Intervention (CMI program and services provided)
  • Monitors and ensures staff safety in agency and in the field when providing Crisis Mobile Intervention Team Services
  • Reports to Associate Director regarding potential and safety issues with staff
  • Supports the Associate Director in implementing and enforcing procedures to maximize safety for consumers and staff served
  • Models safe behaviors for other staff members to see and emulate
  • Supports the Associate Director in implementing and enforcing policies and procedures in accordance with best practices and appropriate standards of care for service delivery model
  • Ensures staff compliance with agency guidelines/procedures governing services provided through coordination with the Associate Director
  • Models adherence and compliance to agency guidelines/procedures for other staff members to see and emulate
  • Supports the Associate Director in overseeing staff scheduling (as needed) to assure 7-day/week/365-day per year coverage
  • Provides backup support and assists with weekend and evening coverage as necessary and appropriate
  • Attends Catholic Charities Meetings when asked or required by the Associate Director and/or North Central Behavioral Health Director
  • Supports trained workforce to staff Mobile Crisis Intervention
  • Assists in interviewing personnel, participates in making recommendations for hiring new staff
  • Orients new staff and provides training to all incoming and existing staff members in support of Associate Director
  • Supports new employees in skill development and goal attainment during the orientation process
  • Provides appropriate and frequent performance feedback to Associate Director for all Certified Peer Counselor and Case Management Staff
  • Assists in keeping staff informed of agency, program, and regulatory body changes that impact service delivery
  • Assists Associate Director in tracking annual training on topics related to care/treatment of mental illness, CPI, first aid, safety, substance abuse, and management of patient behavior, ethics, as well as, other topics necessary to assure competent service delivery by staff
  • Assists in facilitating team meetings as needed to promote team planning for individuals served
  • Identifies needs for individual training and performance plans with the Associate Director related to staff as needed
  • Coaches and provides performance feedback to facilitate staff development; contributes information to staff evaluations through report to the Associate Director in a timely and thorough manner using agency performance review protocols
  • Supports and models engagement with individuals with serious mental illness and encourages staff members use of appropriate treatment options and recovery supports to address and remove potential obstacles to client recovery
  • Provides feedback to the Associate Director on staff adherence to scope of practice within the Mobile Crisis Intervention Team (MCI) program.
  • Communicates in a professional manner that promotes team work, dignity and a respectful environment.
  • Creates and retains positive relationships and promotes trust and respect
  • Advocates for persons served when the need exists; promotes and teaches advocacy skills to staff when needed
  • Contributes to the development and implementation of quality services consistently looks for better ways to perform job duties, make appropriate changes and participates in relevant agency quality improvement projects
  • Assists the Associate Director in creating structures, systems and processes that encourage successful innovation in delivering least restrictive mental health services to individuals with serious mental illness at risk for hospitalization or experiencing individualized self-defined crisis
  • Coordinates with staff in other programs to ensure coordination with the agencys crisis and/or outpatient services
  • Works closely with the Hospital Liaison, Crisis Services, Supportive Housing, PACT and Outpatient teams to ensure appropriate care alternatives are identified and implemented whenever possible to achieve client symptom stabilization and decrease high frequency Emergency Department and other more expensive services

This position normally requires that physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements. Non-Physical Requirements:

Education

  • High School Diploma, required
  • Bachelors Degree in Mental Health, Counseling, Social Work or related field, preferred

Experience

  • 2 years experience in Mental Health with High School Diploma
