As a member of the Leadership Team perform clinical and administrative duties in support of residential and therapeutic services for adults with SMI, TBI, and dual-diagnoses. Support the implementation of treatment plans, ensure services are provided in accordance with all applicable policies, procedures, laws, regulations and contract requirements, and build an environment which promotes growth for each member of our team and each individual in our care.

Support a clinical team of Counselors, medical records, assistants and peer supports to ensure the goals and vision of Orion Homes meet the needs of all clients.

The Care Coordinator provides ongoing case management support to Residents in Orion’s Behavioral Health facilities. You are Orion’s primary liaison with outpatient clinical teams, and participate in scheduled staffings along with Orion’s House Manager and Program Coordinator. You will work collaboratively with Orion’s clinical team to engage, educate, communicate, and coordinate care with clients, their family, behavioral health, medical and dental providers, community resources and others. You will document all changes recommended for the individualized service plan, and ensure updates are completed by Orion’s clinical team. You will gather data regarding current treatment progress to present at future treatment team

meetings/staffings.

You will perform case management functions including outreach and follow-up, staffings, reviewing assessments, participating in the service planning process, and identifying the need for further or specialty evaluations. You will ensure your work products are added in a timely manner to the Resident’s comprehensive clinical record, including clinical documentation (assessments, provision of services, coordination of care, discharge planning, etc). You will provide evaluation and recommendations on the effectiveness of treatment through the ongoing assessment of the Resident and input from the Resident and relevant others resulting in modification to the service plan as necessary.

Duties include: Serve as part of Orion’s interdisciplinary clinical team, which includes LPC, LAC, LCSW and RN-licensed employees. Review, monitor and evaluate behavioral health Treatment Plans in accordance with mandated timelines, document individual progress and concerns, and recommend appropriate referrals to community agencies. Monitor Orion’s treatment requirements, to include: clinical engagement, frequency of contact, work with informal support system, and external services. Schedule and facilitate treatment team meetings and program visits with outpatient clinical teams and family members. Maintain communication, coordination, and connections with all clinical team participants. Coordinate behavioral health and physical health services with outside care providers. Ensures implementation of referrals. Follow-up on and track referrals for additional community services, providing updates in future meetings. Communicate with others within and outside Orion to ensure coordination and follow-through for Residents needing outside care services. Research community for available resources to meet identified needs and to inform member of resource options. Understand and navigate the rights of the patient regarding access to treatment, appeals, and complaint processes. Willing to commute to conduct onsite visits at our residential homes to develop relationships and discuss services.

Knowledge, Skills & Abilities: Working knowledge of common and uncommon mental & behavioral health disorders, including Schizophrenia, Bipolar Disorder, Major Depressive Disorder, PTSD, Personality Disorders, Substance Abuse Disorders, etc., and associated terminology. Knowledge of methods, procedures, and techniques used in behavioral health Treatment Plans. Knowledge laws and ethical standards relating to the care and assistance for individuals with SMI, developmental disabilities and other disorders. Knowledge of principles and practices of case management. Interviewing and psychological assessment techniques. Psycho-educational and psychotherapeutic principles, practices and techniques in the treatment of behavioral health issues. Best practices in the assessment and treatment of behavioral health problems. Management of behavioral health crisis including risk assessment, crisis intervention techniques, crisis resources, and hospitalization protocols. Skills in developing, evaluating, and maintaining member service plans. Skills in problem identification and resolution. Good communication and interpersonal skills as applied to interaction with a diverse team of co-workers, supervisors, management, and outside collaborators. Ability to follow direction, use of sound judgment and excellent time management skills are essential. Must maintain good recipient relationships, be able to identify priorities and stay organized. Knowledge of community-based behavioral health services.

Qualifications: Minimum High School Diploma and quality experience in the behavioral-health field; Bachelor's Degree in a human services field preferred. Ability to acquire a Level 1 Fingerprint Clearance Card, without restrictions. Reliable Transportation, AZ Driver's License, Valid Auto Insurance and clean 39-month MVR.
