Managed Care Patient Navigator

As the Managed Care Patient Navigator, you will be a part of a team who provide care for those who need it the most. The Managed Care Patient Navigator is responsible for making contact with patients assigned to the health Center utilizing a variety of methods with the purpose of improving health outcomes through patient engagement. The Managed Care Patient Navigator works collaboratively with other team members to ensure patients receive exceptional service and care and getting connected to a Primary Medical Home. Primary Functions:

  • Make personalized, outbound calls to patients attempting to engage them in healthcare and schedule appointments.
  • Make reminder appointment calls, contact patients that have missed scheduled appointments.
  • Answer telephone calls in a professional and timely manner and be responsive to patients who leave messages.
  • Work cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service
  • Prepare and distribute mailings to patients
  • Assist in preparation of daily and monthly reports
  • Work independently or within a team environment
  • Effectively work with other healthcare team members to ensure coordination of care
Successful Candidates Will Have:
  • High school graduate or G.E.D.
  • Excellent organizational skills
  • Excellent communication skills and phone etiquette
  • 1+ years customer office experience or patient experience
  • Experience with an electronic medical record; Intermediate computer proficiency (multi-task through multiple applications including Microsoft Outlook, Excel, and Word)
