Key Responsibilities
  • Patient Care: Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.

  • Quality Assurance and Compliance: Ensure compliance with healthcare regulations, standards, and best practices, particularly those specific to Federally Qualified Health Centers.
Requirements
  • Nursing degree from an accredited program (e.g. LPN, ADN, RN)
  • Licensed in the State of Missouri as a Licensed Practical Nurse or Registered Nursing
  • Minimum of two years' experience in an acute care or primary care setting
  • Basic Life Support (BLS) Certification
Preferred Qualifications
  • Experience with chronic care or patient care management
  • Ability to multi-task and handle a variety of demands
  • Experience with Microsoft Office products
  • Experience with eCW
