Job Summary:

The Nurse Care Coordinator is a key member of the Mercy team, responsible for providing exceptional patient care and support. This role requires a strong nursing background, excellent communication skills, and a passion for delivering high-quality patient outcomes.

Key Responsibilities:

  • Assess patient care requirements: Evaluate the needs of individual patients and make recommendations for care based on severity of illness, intensity of service, and skill level.
  • Coordinate patient care: Work with bedside nurses to ensure patients receive timely, safe, effective, and high-quality care.
  • Lead and mentor: Provide support, guidance, and mentorship to direct and indirect care providers, promoting professional conduct and adherence to regulatory guidelines.
  • Manage patient population: Oversee the care of assigned inpatient and outpatient populations, facilitating an effective process of the new care model.
  • Discharge planning: Coordinate patient discharge planning for post-acute services, length of stay (LOS) management, and utilization management.
  • Collaboration and communication: Work collaboratively with healthcare teams to optimize performance and adoption of best practices.

Qualifications:

  • Education: Bachelor of Science in Nursing preferred.
  • Licensure: Current licensure in the state of employment.
  • Experience: 2-3 years nursing experience.
  • Certifications: BLS and ACLS within 90 days of hire/transfer.
