The Lead Navigator for Transgender Care plays a pivotal role in assisting the Director of Gender Affirming Care and the Trans Care Navigators at DAP Health in delivering tailored support to transgender and gender expansive medical patients.

This position is dedicated to enhancing patient engagement in healthcare, particularly in facilitating referrals that lead to specialized care for gender-affirming surgeries and related services.

The Lead Navigator possesses extensive knowledge of the policies and procedures that govern the surgical referral process and the operational framework of the Trans Care Navigation team.

As a primary resource, the Lead Navigator provides procedural guidance to Trans Care Navigators, helping them navigate the complexities of healthcare systems both within DAP Health and with external specialists.

The Lead Navigator fosters a supportive and empathetic relationship with clients, aiming to empower them and enhance their understanding of health systems, while also facilitating access to DAP Health services and community resources.

This role involves collaboration across various departments within DAP Health and with external organizations to ensure seamless service delivery, coordinating efforts within the multidisciplinary care team and with outside specialists.

The Lead Navigator will also be responsible for training Trans Care Navigators as needed, developing peer supervision projects, managing departmental task scheduling, and representing the department in relevant care team meetings.

Activities may be conducted in various settings, including virtually, within health centers, administrative offices, and community environments.


Compensation:
$22.38 – $27.08 per hour

Primary Role-Specific Duties:


Collaborate with supervisors to establish departmental Key Performance Indicators (KPIs) and develop frameworks for effective reporting and tracking.

Oversee the Trans Care Navigator team, managing workflows and the referral process, while working as part of a multidisciplinary team to ensure access to gender-affirming surgery.

Facilitate the timely and accurate processing of referrals for gender-affirming surgeries, guiding patients through both internal and external processes, including insurance navigation and documentation preparation.

Assist in obtaining necessary authorizations for surgery referrals and support patients in scheduling appointments with both DAP Health and external specialists.

Provide information to patients and community members regarding common referral partners and insurance processes, encouraging patient-directed research in pursuit of surgical goals.

Establish and maintain relationships with providers and insurers, facilitating communication between external parties and patients as appropriate.

Guide patients through both medical and non-medical services at DAP Health and external agencies, acting as an advocate and providing referrals as necessary.

Maintain regular contact with patients to develop individualized, patient-centered care plans.
Manage patient expectations regarding health goals and clarify available support at DAP Health.
Assist patients in establishing care to promote long-term retention and address barriers to access.
Keep accurate and up-to-date records in the electronic medical record (EMR) system or other data systems, adhering to departmental standards.

Follow up with patients identified as at risk of disengagement from care or who have not been seen recently.

Participate in quality initiatives in collaboration with the Population Health Quality team to enhance documentation and care for transgender and gender-expansive clients.

Maintain a comprehensive list of resources and gender-affirming specialty medical providers.
Support the creation of educational materials, including patient-facing handouts and social media content for DAP Health.

Engage in ongoing professional development through regular staff meetings and collaboration with other teams or stakeholders as directed.

Document all work activities in accordance with departmental standards.
Assist in data collection and report writing for funders and other internal or external partners.
Additional responsibilities may be assigned as needed.

Qualifications:
Minimum
Qualifications:


A minimum of 2 years of relevant college-level education is strongly preferred; those without formal education should possess equivalent experience.

A minimum of 5 years of paid work experience providing direct assistance to transgender and gender expansive communities in public health, healthcare, human services, or similar settings.

Experience in providing HIV prevention, linkage, or retention services is preferred.
2 years of experience working with a diverse population and the ability to collaborate effectively with varied groups is required.

2 years of experience engaging with members of the Lesbian, Gay, Bisexual, Transgender, Gender Expansive, Queer, Asexual, and Intersex communities is preferred.

2 years of experience working with individuals living with HIV or related issues is preferred.

Special Conditions of Employment:


All staff currently working within a clinic must maintain a current and valid CPR and First Aid certification on file with Human Resources.

This requirement applies to both clinical and administrative staff.

This certification must be obtained through the American Heart Association, as internet certificates are not valid for compliance.
