Requisition ID: 2648 SUMMARY Provides outreach support, linkage and advocacy for consumers at the Cleveland County Jail. Provides Case Management services to Adults, Youth and their families within the community and Red Rock BHS programs. Care Managers interact regularly with other staff, including the Health Team, and treatment providers to assess and treat individuals with mental, emotional, or substance abuse problems, including abuse of alcohol tobacco, and/or other drugs. Activities may include crisis intervention, case management, treatment planning, client advocacy, prevention and education. Provide supportive individual and possibly group rehabilitative treatment services. Works extensively with external partners such as the Court system, DHS, Social Security Administration, families of consumers, other CMHCs, state hospitals, private hospitals, crisis centers and other treatment providers. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Serves as linkage for consumers by coordinating their services within Red Rock BHS and attending to their various Case Management needs in the community Monitors consumers' individual progress in various programs, such as the Mental Health Court Program and makes changes to their Treatment Plan as needed Serves as linkage between Red Rock consumers and various external state and city staff, advocating on consumers' behalf and holding them accountable to the Program standards Coordinates approved services, facilitating communication between various service providers with DHS or other referral sources; advocates on behalf of each consumer to ensure that treatment and reintegration plans are being followed, and assists the consumer, family, and referral source in arranging follow up support services Serves as direct linkage to inpatient hospital staff where a consumer is placed by meeting with staff to develop a specific discharge and aftercare plan Serves as linkage for consumers receiving mental health treatment at Red Rock with other community resources including financial, housing, community, dietary, health and daily living skills; referral linkage, and advocacy; acts as liaison when required Serves as linkage with other Red Rock programs concerning consumer care including Med Clinic, therapy and diagnostic services Prepares, plans, facilitates, and implements direct care of consumers through groups, individual supportive rehabilitation and supportive counseling services; may arrange for appropriate placement in groups and reviews progress Provides crisis assistance to assure safety of consumers by consultation with consumer, staff psychiatrist, family members, and area hospitals/emergency units; provides proper paperwork for hospitalization Completes all services plans and completes prior authorizations in a timely manner; provides and completes all paperwork assignments Participates in disposition/treatment team meetings providing information pertinent to team treatment of consumer Maintains core competencies in relation to working with co-occurring disorders through continuing education and implementing skills into all aspects of treatment Attends staff meetings, workshops, and seminars to learn agency policy, rules, regulations, and procedures; participates in on-going in-service training Assists in the development and implementation of programmatic goals and objectives Maintains confidentiality and informs consumers of rights and obtains necessary authorization for obtaining and releasing information Builds rapport and working partnerships with relevant local, state, and federal government agencies Promotes community education projects to help increase awareness of Post-Traumatic Stress Disorder (PTSD) & Acute Mental Health Crises Supports and assists with the facilitation of Care Plans for consumers within the Red Rock Health Team Performs other duties as assigned QUALIFICATIONS Two or more years’ experience working with community services and health agencies as well as working with low income and minority consumers with understanding of cultural diversity preferred Current certification as a Behavioral Health Case Manager II (BHCM II) OR Bachelor's degree in approved mental health related field General skill in exercising independent judgment, tact, patience, and understanding; maintaining effective working relationships and strategic alliances with consumers, other staff, the community and other non-profit organizations Must complete all in-service and external training requirements Basic computer skills Valid Oklahoma Driver License, with a clean Motor Vehicle Record, and current Auto Insurance Commitment to the mission of Red Rock BHS BENEFITS (Full-Time Staff Only) 95% Employer Paid Health Insurance Plan Vision Insurance Some Positions Qualify for NHSC Student Loan Repayment 403B Retirement Plan with 5% Employer Contribution Annual Professional Growth Funds Employer Paid Life Insurance and Long Term Disability Equal Opportunity Employer/Protected Veterans/Individuals with Disabilities Red Rock Behavioral Health Services does not discriminate based on race, color, national origin, religion, gender, gender identity, age, marital/familial status, sexual orientation, or disability.
