The selected candidate serves as the on-site clinical leader at FHCHC's Harm Reduction Center. This clinician provides high-quality medical care to individuals seeking services at the Harm Reduction Center in a manner that is consistent with Fair Haven Community Health Care (FHCHC) policies and mission. Upholding the principles of harm reduction, the clinician will use a recovery-oriented approach to all individuals seeking such services. Duties and responsibilities + Provides primary medical care, including urgent care and primary care, for adult patients + Appropriate offers and prescribes medications for opioid use disorder (MOUD) and medications for alcohol use disorder (MAUD) as clinically indicated + Appropriately offers and prescribes pre-exposure prophylaxis (PrEP) for HIV prevention as clinically indicated. + Develops a plan of care for each patient, including medical history, physical examination, diagnosis and appropriate treatment and/or referral, including hospitalization if necessary + Uses all available resources in diagnosis and treatment, such as laboratory and others tests in an appropriate and cost efficient manner + Confers with other clinicians, patient's families and other support staff concerning treatment and care of patients when appropriate + Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care + Participates in FHCHC's Quality Improvement program, in other task committees as needed, and pursues continuing education opportunities to update medical knowledge and assure quality patient care + Performs other necessary duties as required by the FHCHC to achieve the goal of providing high quality service and health care. + Provides primary medical care, including urgent care and primary care, for adult patients + Appropriate offers and prescribes medications for opioid use disorder (MOUD) and medications for alcohol use disorder (MAUD) as clinically indicated + Appropriately offers and prescribes pre-exposure prophylaxis (PrEP) for HIV prevention as clinically indicated. + Develops a plan of care for each patient, including medical history, physical examination, diagnosis and appropriate treatment and/or referral, including hospitalization if necessary + Uses all available resources in diagnosis and treatment, such as laboratory and others tests in an appropriate and cost efficient manner + Confers with other clinicians, patient's families and other support staff concerning treatment and care of patients when appropriate + Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care + Participates in FHCHC's Quality Improvement program, in other task committees as needed, and pursues continuing education opportunities to update medical knowledge and assure quality patient care + Performs other necessary duties as required by the FHCHC to achieve the goal of providing high quality service and health care. Qualifications + Graduation from an accredited APRN or PA program; license to practice medicine in the state of Connecticut; ability and willingness to serve on the active medical staff of the hospitals used by FHCHC; qualified in Basic Life Support; experience prescribing MOUD, MAUD, and PrEP. + Bilingual (English/Spanish) is preferred. Manual Dexterity Required: Ability to use a keyboard, telephone.
