Job Summary
Home Care Agency is seeking to hire a community outreach representative to promote and build our company within the Brooklyn/Queens area. You will be responsible for establishing and maintaining relationships within the community in order to generate quality leads for Home Care services.

Essential Duties and Responsibilities:
  • Responsible for Submitting Daily Reports to Management.
  • Building relationships, receiving referrals, and promoting our agency with Hospitals, Nursing Homes community organizations, and physician practices.
  • Managing and creating new referral sources (nursing homes, hospitals, community referrals, etc.)
  • Conduct presentations to support groups, adult day cares, Community Events, etc. as well as conduct home visits, and personalized appointments as needed to complete the enrollment process.
  • Follow up on leads and referrals from family members, caregivers, and other sources.
Qualifications and Skills:
  • Must have a minimum of 1 year of Sales experience.
  • The ability to hold events and build relationships within the community.
  • Good Communication Skills
  • Must be a go-getter and self-motivated.
  • Must have a car and be able to travel to meetings daily.
  • **Preferred Bilingual: Fluent in Spanish & English
