The QIDP/Intake Coordinator will carry a QIDP caseload within the Community Living Program. Along with focusing on admissions for the program (staff, and individuals with intellectual and developmental disabilities) to support.

  • Manage a QIDP caseload within the Community Living Program. This will include, but is not limited to, attending bi-annual Life Plan meetings for individuals on caseload, update Staff Action Plans at least twice annually or as needed, review the progress of individuals as needed, and maintain familiarity with both internal and external requirements for compliance.
  • Participate in agency and program training as it relates to tasks assigned to agency QIDPs
  • Meet regularly with Assistant Director of Community Living on progress of intake and coordination of individual-staff match-ups in Community Living programs.
  • Will communicate with the Care Coordinators on matters relating to the development and progress of the Life Plan. Will ensure quality services and positive outcomes.
  • Will communicate with families/advocates and other interested parties in matters relating to Community Habilitation services, including resourcing the community for recreational and social activities.
  • Will assume program responsibilities in the role of QIDP. This will include coordination and oversight of staff and family support meetings, development and writing of Community Habilitation Staff Action Plans for each individual, participating in semi-annual reviews, and reviewing the progress of each individual as needed.
  • Assuming QIDP responsibilities will also include reviewing and maintaining all required documentation in NX.
  • Provide support with weekly billing (reviewing and approving notes), when needed.
  • Assistance with EVV (Electronic Visit Verification) support in reviewing billing sessions, tablet support, and monitoring system portal, as needed.
  • Assist with intake of individuals wishing to receive Community Habilitation, Respite, After School, and Self-Direction services from Living Resources Community Living Department. This will include, but is not limited to, meeting with interested participants and families, sharing information with Community Living managers and Assistant Director, and assisting with coordination of meetings between staff and individuals.
  • Work cooperatively with Human Resources recruiting staff to coordinate appropriate matches between staff and individual in Community Living program services such as Community Habilitation, Respite, After-School Program, and Self-Direction.
  • Manage the admissions and discharge process for individuals receiving supports and services including completing and maintaining all intake/discharge documentation. This will require understanding each program and necessary admission/discharge requirements
  • Knowledge of Medicaid, Medicare, SNAP and Social Security benefits and applications/certifications for each
  • Gather necessary documents for all new admissions
  • Ensure changes to individual's service authorizations and amendments are submitted to OPWDD
  • Collaborate with the finance department and benefit specialists to troubleshoot issues impacting billing and cash flow
  • Ensuring all Medicaid and waiver documentation is in place prior to admission and submit documentation for enrollment in NetSmart.
Requirements
  • Bachelor's Degree required
  • at least 2 years working with individuals with intellectual disabilities
  • Valid NYS driver's license
  • Complete all state and agency required training
  • Excellent verbal, written, interpersonal and communication skills
  • Knowledge and understanding of local and regional regulatory operations
  • Knowledge and understanding of OPWDD regulations
  • Understanding and commitment to community-based support for persons with disabilities
  • Experience with intake and admissions
  • Excellent time management skills
  • Proficient in Microsoft Office (Outlook, Word, and Excel)
