Job Title: Case Manager

Join Equinox, a non-profit human services organization in the Capital Region, as a Case Manager in our Transitional Living Program. Our program provides a supervised group residence and apartment living for homeless youth, ages 16 to 21, to help them make a successful transition to life on their own.

Job Summary:

Under the general supervision of the Program Manager, the Case Manager will be responsible for delivering services to youth in the Transitional Living Program. This includes developing trusting relationships with program youth, teaching independent living skills, maintaining clear documentation, and making referrals for additional community services.

Key Responsibilities:
  • Develop trusting and supportive relationships with program youth
  • Teach youth independent living skills, including educational, vocational, financial, housing, social, and psychological skills
  • Maintain clear, concise, and current documentation in electronic and paper case records
  • Interview potential applicants for the residential program and make recommendations for admission
  • Work with assigned youth to develop and amend individual service plans
  • Make referrals for additional community services and follow up as necessary
  • Monitor youth attendance at educational, employment, and healthcare commitments
  • Provide advocacy assistance and attend court with youth as needed
  • Participate in Transition Plan and Service Plan Review Meetings
  • Maintain confidentiality and provide conflict resolution assistance between roommates and other program participants
Requirements:
  • Bachelor's Degree in human services or related field
  • At least 2 years of experience working with youth, preferably in a residential setting
  • Valid NYS driver's license and clearance by the NYS Central Register of Child Abuse and Neglect
  • Ability to lift 20 pounds and perform life skills, such as cleaning and stairs
