Job Title: RN Case Manager

St. Peter's Health Partners is seeking a skilled RN Case Manager to join our dynamic healthcare team. As a member of our team, you will have the opportunity to make a meaningful impact on the lives of our patients and their families.

Job Summary:

We are looking for a highly motivated and organized RN Case Manager to collaborate with our healthcare team to manage the progression of patients' care. The ideal candidate will have a strong background in nursing and experience in case management or utilization management.

Responsibilities:
  • Collaborate with the healthcare team to develop and implement individualized care plans for patients.
  • Conduct thorough assessments of patients' needs and develop plans to address any gaps in care.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care.
  • Provide education and support to patients and families to promote healthy behaviors and self-management of chronic conditions.
  • Work collaboratively with the interdisciplinary team to ensure optimal patient outcomes.
Requirements:
  • Current and valid NYS RN license.
  • Bachelor of Science in nursing, or willingness to pursue defined plan for matriculation in program demonstrated by ongoing coursework each semester.
  • 2 years of experience in acute setting.
Preferred Requirements:
  • 1+ year of experience in case management or utilization management.
  • Case Management certification (CCM) or Certified Professional in Healthcare Management (CPHM).
  • Discharge planning and community resource knowledge.

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
