General Description of Duties:Partner with schools, case managers, families, and State/Federal agencies to develop a client referral system statewide.Determine the funding needs of each client, if applicable, and make appropriate referrals.Provides transition support to the school systems throughout the State to include colleges.Serves as a liaison to schools, case managers and State/Federal agencies in the recruitment of clients for Adelante programs and contracts.Provides support to the disability community as a resource about Adelante programs.

Education and Experience:High school diploma or GED.Four (4) year college degree strongly preferred.Experience working with a school to work transition program is desirable. Midlevel experience working with necessary word processing, and spreadsheet software (Microsoft Office, Word, Excel, etc).

Other Requirements:Must pass pre-employment drug and TB screen, and Agency, Federal and State Department of Health background investigations.Additional background investigations may be required depending on the job assignment.Must have and maintain a valid NM driver's license.Must have own, reliable transportation and an insurable driving record. Job Type: Full-time Monday thru Friday 8am - 5pm.

Pay: 45-50k annually DOE.
