Location: Remote

Duration: 3+ Months

Pay-Rate: $25.00/Hr. on W2

Position Overview: As an Administrative Care Management Support, you will play a crucial role in supporting care management activities by performing outreach, answering inbound calls, and scheduling services. You will serve as a primary point of contact for members, providers, and internal staff, addressing inquiries, resolving issues, and documenting member interactions accurately and in compliance with state and regulatory requirements.

Key Responsibilities:
  • Conduct outreach to members via phone to assist with care plan next steps, provide information on community or health plan resources, and address questions or concerns related to scheduling.
  • Support members by connecting them with appropriate health plan and community resources to ensure comprehensive care and service delivery.
  • Utilize working knowledge of assigned health plan activities and resources to effectively support member needs and inquiries.
  • Serve as the initial contact for member and provider inquiries, explaining care plan procedures, protocols, and benefits as needed.
  • Assist in member onboarding processes and perform daily administrative tasks such as sending welcome letters, related correspondence, and educational materials.
  • Document and maintain accurate non-clinical member records in accordance with state and regulatory guidelines, ensuring confidentiality and compliance.
  • Possess knowledge of available local benefits and resources, making referrals to address Social Determinants of Health (SDOH) needs as appropriate.
  • Perform additional duties as assigned to support the efficient operation of care management services.
Requirements:
  • High School diploma or GED required.
  • 1-2 years of related experience in customer service, administrative support, or healthcare coordination preferred.
  • Strong interpersonal and communication skills, with the ability to effectively engage with members, providers, and internal stakeholders.
  • Familiarity with healthcare terminology, care management processes, and regulatory requirements is advantageous.
  • Proficiency in documentation and record-keeping, ensuring accuracy and adherence to standards.
  • Ability to prioritize tasks, work independently, and collaborate effectively within a team environment.
  • Commitment to upholding company policies, standards, and ethical practices.
